We understand that arriving at an Integrated Medical office for the first time can be nerve racking. If you've never experienced Integrated Medical care before, you may not know what to expect when you come to our office for the first time. Filling out paperwork, learning new faces, and getting to know your way around the office can be an overwhelming experience.

Online New Patient Portal
ReNew Medical Center, LLC utilizes an online patient portal for new patient paperwork. Once your appointment is scheduled, you will receive a link to set up your profile on the portal, then you will be assigned new patient paperwork to complete at least 24 hours before your appointment. Completing the paperwork in advance will save you and our office staff valuable time, so please be sure to complete all the required forms prior to your appointment.
